It won't repair one of the more common problems, improper file and folder permissions, but that's what Apple made Drive Utility for. Drive optimization is another issue. There are quite a few files that need to remain "anchored" and "untouched" that just don't exist in OS 9 or earlier, so I would avoid using any disk optimizer or defragmenter not specifically designed for OS X.
