Have you checked to make sure your SCSI ribbon connector is firmly seated in place on the motherboard connector? Is the end of the SCSI device chain terminated? How are you attempting to boot from CD ... holding down the "C" key? That doesn't work on a lot of stock Umax CD-ROMs so you might want to try the "option-command-shift-delete" combination which will try to boot from the next device # down the SCSI chain. It sounds like you know what your doing with zapping the PRAM and resetting the CUDA after each hardware change ... if all else fails try one of the Supermac list's tried and true "magic" solutions ... ;) Pull the battery and physically unplug the computer, preferably leave overnight. The next day, make sure everything is connected/setup properly, put the battery back in and restart. (Note: some people claim you only have to leave it for 10 minutes or so, but I left it overnight just to make sure.) I was a disbeliever until I came across a problem that just couldn't be fixed with the all the usual troubleshooting techniques ... this one raised my computer from the dead more than once!
