I have seen it happen. I think the culprit is the fact that the relevant
folder changed names. Sometimes the conversion process is not entirely
successful and the entries aren't moved from the old folder (I think it
was called "Bookmarks Toolbar" or something like that) to the new
"Personal Toolbar" folder.

You can open the Bookmarks Manager and move them easily, though.
