It reacts when links in the message are misleading -- for example, if the display text is <http://www.areallycoolsite.com> but the link actually points to <http://www.phishingcentral.com>. You'll get that when the newsletter uses a service like Constant Contact, where all the links go through the service so they can track who clicked them. Mouse over the links and you'll see what I mean in the status bar at bottom left.
