Most of the information you are looking for is in the file 
solicit_.103. Using FileMaker, log in with the master-level password, 
and you could export directly from that file. You'll want the fields 
Source Code, and Source Code Description certainly. The fields 
Response# is the number of payment records with that source code (at 
least those included in the relationship Gift, which means Dues and 
Contribution payments. Event\Purchase payments are not included. 
Action# is the number of Action records with that source code. There 
are no fields at present to accumulate Contact records, but you could 
add one using Action# as a guide.

I'd export the records to an excel sheet so you could sort, format 
and print them to suit.
