I can't remember the specifics, but it sets one of the top bits of the
zeroth FAT entry (never used because the FAT itself is there).  I think
there are two special bits, one signifies improper shutdown with open files
on the disk and the other a disk error (which I believe prevents Windows
scanning or using the disk).  It is possible to write a .COM to reset the
flag every bootup, but I just use TweakUI and turn off the automatic check.
