I have had exactly your problem with mono on color monitor.
Video card worked with my monitor for years, then went mono
after a power failure.
My symptoms were these:
1) boots normally, runs DOS games (e.g. Wolfenstein, etc.)
   in color mode, no problem.
2) runs non-game color apps in b&w
3) when starting Win31 displays splash screen in full color
   but once windows starts, reverts to mono.

Evidently there is a something in the video card that can
be TOLD to go to color, and then works, but which won't
DETECT that the monitor is color.
I had to replace that video card, I recommend you do also.
