My slight variation on Or's procedure (from a DOS window):
0) format as many diskettes as you will need
0) format them twice -- modern media is flaky
1) SYS A:
2) copy the most basic setup files to the first disk
FDISK.EXE ... needed to create the partition(s)
FORMAT.COM ... needed to make partitions usable
SYS.COM ... needed to make partition bootable
HIMEM.SYS ... invoke in CONFIG.SYS for UMB access
EMM386.EXE ... invoke in CONFIG.SYS for EMS/XMS
XCOPY.EXE ... copy files and or directory trees
DOSKEY.EXE ... edit recent DOS command lines
MODE.COM ... set up ports, keyboard rates, etc
EDIT.EXE ... or other small editor of your choice
EDIT.HLP ... for the novice EDIT user
* note, I prefer TED.COM and/or QE.EXE for my editor
CONFIG.SYS ... a sample one to modify
AUTOEXEC.BAT ... a sample one to modify
(and other files as there is room)
3) copy whatever doesn't fit on this disk to the other
formatted disks, including (if you have one) a decent
file and directory manager
CHOICE.COM ... prompt user for option in batch file
DELTREE.EXE ... remove portions of directory tree
MEM.EXE ... display memory usage and available
MORE.COM ... view long text files a screen at a time
MOVE.EXE ... move files/directories in tree
TREE.COM ... display directory tree structure
* DMLITE.EXE ... lightweight disk manager
* allows easy moving, copying, deleting, viewing of files
* (if you can't find it, I have it)
PKZIP.EXE ... file compression utility
PKUNZIP.EXE ... file decompression utility
* (no, these don't come with DOS or Windows)
... and other things you may want.

And now, on the destination machine Hard Disk Drive (HDD)
4) if HDD is not yet prepared, run FDISK
5) if HDD is not yet formatted, run FORMAT C:
6) if HDD is not yet bootable, run SYS C:
7) on the new C: drive,
MD DOS ... create DOS directory
CD DOS ... change to DOS directory [ C:DOS> ]
8) for each floppy
COPY A:*.* C:
9) in your new AUTOEXEC.BAT make sure you have a line
that reads
SET PATH=C:DOS
(or at least contains an entry for C:DOS)
Optional material:
Now, additionally, I have a bunch of DOS utilities to make
running a machine more fun. I use QuikMenu as a GUI way
of selecting programs. It uses NO runtime memory -- it's
just a launcher. Nicely done. I also use XTREE GOLD 3.0
which has built-in PKZIP 2.04g support (I use its clone,
ZTREE on my Win32 machines -- excellent hack).
Then there's all the old PC Mag tools, and others that
the other List members will know about.
And finally, if that's not enough, I have a bootable
image for the text version of Partition Magic, if you
need something stronger than FDISK for your new drive.
I've had to do this a time or two on machines where we
got Windows9x with the machine, but needed to run some
application or networking software that fought with
the Windows GUI, yet we still wanted access to the
whole drive.
It was comical. We had to install Windows, create the
diskette set, then use the new diskettes to burn down
the Windows partition and create a DOS-only environment.
