I used to use MS-DOS 4.01.  Before there was MS-DOS 4.01 there
was an MS-DOS 4.0 which appeared but briefly and suddenly
discontinued because of some alleged bug.  People who had it were
advised to turn it back in and to apply for the 4.01 upgrade.
Any editions of 4.0 which might still be in existence are
probably rare collector items.  I don't know any details about
this bug that was supposedly present in the first release of the
major version upgrade.  I don't know anyone who ever used DOS 4.0.
Most people including myself didn't like MS-DOS 4.01.  Memory
management was very poor and the system left a big footprint.  A
lot of people who tried out 4.01 downgraded to 3.30 because 4.01
was so bad and such a hog for memory.  That is what I did.  I
don't think there were any upgrades for the major version 4.x
beyond 4.01.  I never heard about any so-called multi-tasking
capabilities in either of the DOS 4.x releases.  If either of
these releases did in fact have any such capabilities, and if they
worked OK, then the DOS 4.x versions would not have been so
unpopular, especially considering that the earlier DOS version,
3.30, remains very popular even to this day among many users of
SurvPCs.  BTW, I still have my original installation disks and my
user manuals for DOS 4.01.
