manpages: Drop the MANPROGPREFIX variable
Instead have PROGRAM.c and PROGRAM.xml in sources. Actual installed names
determined by contents of PROGRAM.xml.

manpages: Don't assume 1:1 between the man page source and dest file names
Instead rely on refnames.sh for the list of generated names and use
dummy targets to generate things always.
