state: move delete state code to del_state_from_db(); simplify
rehash_state()
When rehashing, simply remove and re-insert the state.
Commit 705b035d652e7091770d370fb6c62b3a98f93936 didn't update
the icookie hash table. While probably harmless, it made
things inconsistent.
