ikev2: merge struct initiate_list into struct v2_msgid_window
Update send code. Only increment UNACK once per iteration from
6429f3dfc817743144d52faa021bd86ac5cc4cad (since window-size==1 UNACK
was only ever 0/>= so the bug is somewhat theoretical). Remember to
free list.
