pluto: use the sighash_policy_bits as a proper lset_t
coverity found "dead code", which showed POL_SIGHASH_NONE was not
properly used. Phase it out in favour of using LEMPTY.
Additionally, ensure proper logging always happens.
Removed "??? what diagnostic?" questions. We don't want to send too many
different NOTIFY payloads back to the other side (as per RC), so we will
end up with NO_PROPOSAL_CHOSEN anyway.
