host-pair: pass local/remote into find_host_pair() by value
Use remote=unset_address and not NULL to indicate a search for
an "unset" aka "any" address - remove any possability of an NPE.
In host-pair's hasher() don't hash a remote's unset/any address
Explicitly handle a connection's remote host_addr being either "unset"
or "any".
