ip: add missing *_from_raw() and pexpect_*() functions
- include where_t
- eliminate range2() and endpoint3()
- enable pexpect in range_from_raw()
- but don't enable pexpect in endpoint_from_raw():
-> acquire creates 192.1.2.45:TCP/0 -> 192.1.2.23:TCP/80
-> orient creates 0.0.0.0:UDP/443
