CRLs: add --impair event-fetch-crls, treat problems as fatal
When --impair event-fetch-crls, don't schedule the 5s delayed initial
fetch. Suspect the delay is to give tests a short window before the
CRL list is refreshed and bad certs are flushed.
And treat things such as CURL or creating a fetch thread failing as
fatal().
