So you need to make sure you give enough room in your swf to display the
skin. As you have specified the "under" skin, ot means you need at least
20px or so of extra height to display the skin under the video. What are the
dimensions of your f4v?

To be clear, everything is displaying correctly when you export the HTML /
Flash file on your desktop? (I am just surprised how much trouble you are
having if everything was already working as intended before you tried to
place your content on your web server)
