For pull requests, you can click on a line number and get an input field to write your comment for that line. I'd have thought that that's possible in gists, too, but I haven't found a way to do so, so maybe you still need to set up a Github repo (create a Github account, fork sympy, git clone the fork to your local disk, commit your changes to the clone and push them to the fork on Github - the whole process is a bit lengthy, but Github comes with a very extensive blow-by-blow tutorial for it so that should work for you).
