The command line is of later worry, I think. This kind of clarification
 that is happening here must precede it. The command line can probably

Fair enough. However, when or if changes to the model are proposed, then
the implications for user documentation and the command line syntax
should also be considered.

That in turn is fair enough, but I'm not currently proposing changes to the model, I'm just documenting the model as it is (note that this is a conceptual model -- the implementation might make various choices that implement such a model that look very different). Regardless of how the were used in communication before, the matrix that Graham sketched appears to be an accurate description of the current situation, regardless of how it is spoken about from any (limiting) perspective. Knowing what lives where is hugely helpful in puzzling out the logic of what actually goes on; it's one of the core features of my HOWTO.
