One possibility: The upgrade relies on files in /var/sadm/system/admin in order to work out what the system currently has installed. If those have been removed, or are inaccessible when booted from the DVD, then you won't see the upgrade option. Look in particular at /var/sadm/system/admin/INST_RELEASE and /var/sadm/system/admin/CLUSTER. One failure is if you've needed to move any of the files or directories under /var/sadm elsewhere to make space; you ought to move them back.
