Every on line forum I've ever been in I've been told that once a SIM  
card has been de-activated it almost always cannot be re-used and  
never for another account.  As I said earlier there may be certain  
situations where a de-activated card may be re-activated, but those  
situations are rare.  A SIM most certainly cannot be re-activated for  
a different account.  If you inquire of T-Mobile they will more than  
likely tell you that you can activate an account using an already or  
formerly activated card.  More than likely it's a security issue,  
though I don't know if that's the case.

I had to get a new SIM card today since my phone was lost or stolen  
three weeks ago before I left the states for three weeks out of the  
country.  A new SIM was given to me at no charge.
