If we use ref, then I feel it should be ref:xxx= since PSMA isn't the
authority and since as far as I can tell PSMA create these ID's not the
state authorities, hence thes ID's aren't the main reference, unlike other
things in OSM using ref.

I don't think it's too hard to get it right the first time, we're already
planning on doing it state by state (though not all states will be
imported, ones that are mostly complete already won't need to be done).
