Afterthought: that application already exists. It's called OpenStreeBugs.
(Even the name is appropriate for reporting holes in the streets ;-))
With slight modifications, it is even better than Fixmystreet:
* it does not embarrass the reporter with a street name just to pin a
notice
* it avoids reporting the same problem dozens of times
OSM proponents could show OpenStreetBug to the right persons as a
convincing starting point.
