They're surveyed by local authorities (paid for by us), who then compile the data into the definitive statement. This can be issued by them under OGL. They are "obliged" to pass the statement (more usually a visual derivation of it) onto Ordnance Survey.

It's OS's lazy/arrogant/corrupt business model which prompted me to start contributing to OSM.
