Hi,
Go to Window-eyes diagnostics from the app menu and run the  refresh 
accessibility libraries option.
After rebooting see if you can alt-tab as expected.

Why is it that I hear nothing when I hold down the alt key and press Tab?
Having to release tab to hear anything is very frustrating, and slows you down 
terribly. Also, it takes several seconds to hear anything after you let those 
keys go.

I hope it isn't a virus!
