I have never used it, but I think another survey tool out there is called PollMonkey. Might be worth trying that. In either case, you need to know if you can use the tool for free. My recollection is that the free survey tools are limited on how many survey responses you can collect. For what you are trying to do, you may not have a free option that will gather the volume of responses you seek, even if you can create the survey.
