I am posting my reply to the list as it might provide value for other list 
members. I just took a quick look at this site. Specifically, the reservation 
page here: http://www.meowparlour.com/reservation/. I assumed this page 
contains the form you were having difficulty with. If not, I suspect the form 
you are trying to use is similar to this one.

In order to display the reservation form, you first have to "click" on a custom 
element that looks like a button but really isn't. If you can find this Book 
"button" in Browse Mode and activate it, then the form is dynamically displayed 
on the same page.

There are three parts in the reservation the form:

1.   Participants (combo box/select drop down)

2.   Date (date picker widget)

3.   Activity (fake buttons)

This form is full of accessibility issues. One of the biggest issues I 
discovered was that the date picker widget and activity buttons aren't operable 
with the keyboard at all out of Browse Mode. So, this form would probably be 
difficult or impossible to fill out regardless of what screen reader you use (I 
did test with another screen reader to verify this). You might be able to fight 
your way through it using advanced screen reading techniques, but I doubt most 
people would have much luck without some assistance.

I would encourage you to send an email or call this business in an effort to 
help them understand that not everyone is able to use a mouse to interact with 
their webpage. And, because the form is inaccessible from the keyboard, they 
are losing business, discriminating against people with disabilities and 
violating Title III of the Americans with Disabilities Act (probably 
unknowingly so try to be nice about it).

Here is a pro tip on how to explain keyboard accessibility to someone who 
doesn't understand the concept. Ask them to unplug their mouse (or not use 
their touchpad) and try to book a reservation on their website. They will 
quickly see the accessibility barrier they have inadvertently created.
