Under Ease of Access, tab until you hear "Change Signin Settings," which opens a dialogue of check boxes pertaining to any screen readers, magnifiers and other accessibility features on your system. Each checkbox applies either before or after the signin screen, which is by design; this way, you can have some things running at login and others after Windows loads. In any case, your Narrator is obviously checked there, which is why it's running at startup. Just uncheck it.
