I haven't been following this thread so apologies if I am off the mark.
I recently installed the latest update for Windows 10. I think it was a
security update. Window-eyes on my desktop then started opening and
then complete silence. I tried using NVDA, Jaws and Narrator but to no
avail. In the meantime my laptop was working fine with exactly the same
update. I then wrote to someone on another list who I believe works for
Microsoft. He advised trying "sndvol" which made absolutely no
difference. After about two weeks my machines both downloaded and
installed another update of some kind. Miraculously, my desktop
suddenly started talking again and I haven't had any further problems.
A Web search found a lot of comments about the update to Win 10 was
causing problems with certain makes of sound card Etc. I have no idea
if any of this is to do with your discussion but I thought the
information might be helpful.

I am one of those who need it.
I have yet to find any other screen reader which allows me to manage the
connection and disconnection of attached devices between the host and
virtual machines on the current version of VMWare Workstation.
Indeed, this is very surprising to me, that is, that I cannot get Jaws or
NVDA or Narrator to focus those device icons, but the truth is before me,
and so I am saved by Window-Eyes.
Also, I have yet to find a screen reader which is as perky reading
email in
Outlook which is part of Microsoft 365.
Microsoft 365 is the new name of Office 365, the subscription-based
Office
product.
The Office Enhance and Outlook Enhance Window-Eyes apps work perfectly to
allow me to read and write in Outlook.
So, Tom, I simply want to suggest that you don't give up on Window-Eyes.

When I figure out how to remove Window-Eyes I'll try
installing it with no apps.
