I think not quoting the very last one is an instance of an old bug which has been too elusive so far to track down. This bug has been around at least since v. 3.0. It happens randomly and, also, for considerable stretches of time will not show itself. Therefore, I say it is random and infrequent. Making it all the worse, because one is lulled into skipping the change to PTV before quoting (the only work-around that I know). In some replies to messages, this omission of a Smiley leads to misunderstandings that range from minor to serious.
