I verified this, it is repeatable here. In 3.62.10 a reply template for someone I do Beta Testing for works. In 3.62.11 it does not see the subject nor the From: address. I switched back and forth from one version to the other. Below, I will offer the offending Header minus a couple of things I munged for whatever reason I munge things.
