I thought the hierarchy was address book entry templates first,
address book group templates second, then folder templates, and
finally account templates. 

In my experience up to v4.0.38, the two or three address book entry
templates I have defined usually take precedence over my account
templates, so long as the outgoing message has no additional 
addressees on the "to" line.

Did you check for folder templates? Additional addressees? What 
happens if you (temporarily) delete the account reply template and 
then reply to one of those contacts?
