  Get into the Account Properties. Look @ the templates. (These are
  your default templates.) If you do not create a template, these will
  be used.

  Click on the ""Reply" template @ the bottom You will see several
  options for "sender information used for quotation". Select
  "Initials" & they should return.

  I don't know why yours disappeared unless you changed this option or
  you are working from a custom reply template which includes a
  different quotestyle.

  As you can see, mine uses your name.
