I currently have around 148000 (yes, that's *thousand*) messages
spread around 150-200 folders and four accounts. No problems with the
database or with indexing.

There is a manual method for forcing a re-index. Exit TB and delete
the messages.tbi file in the disk folder. The on-disk folder hierarchy
mimics that of the account, so it's not too hard to find. On the next
load of that folder (when you restart TB), the folder is re-indexed.
