You have two (or more?) QT's named identically.

The 'secure' way is to rename all file called 'ACCOUNT.QTN' except one,
start TB!, write down QT-names. Stop TB!, rename the last 'ACCOUNT.QTN',
name an other back to 'ACCOUNT.QTN', start TB!, write down QT-names ... and
so on.

Know you should know which QT's have identical names. So you can enable
_one_ (a last time, I promise *G*), start TB! again, rename affected QTs,
stop TB! and rename all 'ACCOUNT.QTN' back. The error message should be
gone next time you start TB!.

An ideal target name when renaming the files can be 'ACCOUNT.QT_' as this
is easy to recognize and change back :-)
