Create an incoming filter that triggers on the sender, if you want the
recipient to be part of the condition too, you press on the 'Add'
button and you can place an additional condition (both conditions have
to be met) as often as you'd like.

Choose as action 'Forward to' (You'll find that if you scroll down at
the 'Actions' tab. Now you can choose a new addressee and you can
define your own template by pressing on the forwarding-template-button
(a letter with a blue arrow) that becomes visible when you've selected
the the forwarding action.
In this forwarding template you can add as much text as you'd like.

Not that I'm aware of. Most templates are strictly personal, though
some of the more complicated macros can be found in the list archives
of this list and tbtech.
