I never tried what you want to do (automatically send a message), but the reading confirmations that I agreed to create were never sent automatically either (though I don't know if they should). I don't see the benefit of TB connecting to Exchange via MAPI (after all, it downloads all the messages, and the Exchange folder and your local MAPI folder will be out of sync in no time, creating duplicate messages in the local folders).
