It seems like you have already found the "This address book is
associated with a LDAP server" check box. The username and password
are whatever credentials you need to log in to the server. On my
server, none are needed.

That really depends on how your LDAP server is structured. I would
contact your LDAP administrator and ask him or her what the proper
search base is.

For the most part, I find that it works well (better than Outlook's
implementation). However, every once in a while, after adding a
contact from the directory, The Bat! does something that something
else doesn't like and goes into a infinite dialog box loop. It keep
popping the same box up again and again. The next time it comes up
I'll file a bug report...
