Help authoring tools should support this for Web-ready graphics (FM+WWP does), and I believe you can use Photoshop and similar image editors to create image maps as well. Image Ready, which is part of Photoshop can be used to create hot spots. I don't know if it does anything beyond rectangles because I use Dreamweaver and Javascript to do that.
