For testing I replaced the \FE and \FEE definitions at the beginning
of your pmx-code by some simpler ones (just some character output
instead) and the "Underfull \hbox" warnings disappeared. Also the
dvi output looked nice (apart from the missing note repetitions of
course).
I didn't have the time to take a closer look yet, but you should try
fiddling with the \FE \FEE defs.
