I partially solved the problems by updating GSview32 to version 4.0,
which also required me to install a new ghostscript (7.03).  The newer
gsview has a more standard windows printer interface, from which I could
successfully set the page size and orientation. (It always seemed wierd
to me that gsview had such an oddball printer interface).  With the new
version, the type1 slurs come out fine.

Flush with success, I thought maybe updating Acrobat would enable it to
deal with the type-1 slurs in a pdf.  So I downloaded and installed
Acrobat Reader 5.0.  But alas, it's a step BACKWARDS from Acrobat 4.0
which I had been using: the long slurs in type-1 fonts now don't show up
at all, either on screen or in the printed page. :-(

I haven't tested this with dvipdfm.
