You're right, some macros are missing. I don't know if they were present in old versions of MusiXTeX of PMX.
But it's easy to reconstitue them. Your example becomes

Similarly, I suppose that the definition of \lwp could be [EMAIL PROTECTED]@width}
I also modified your code to correct the problem of the dot of the note c, which has to be moved to the right (in fact it coincides with the one of the note d)
according to my primary reference for modern music typesetting: Gardner Read, Music Notation (2nd ed. 1969) the below coding would apply
c04d-1.5+1.25 zdd e2 /
I'm specifically referring to Gardner, page 114, concerning intervals of seconds
"If the lower of two notes is on a line, its dot(s) must go in the space beneath it, rather than above"
Gardner doesn't explicitely say that dots of a chord must be vertically aligned. But they are in all his examples.
BTW do you considering updating MusiXTeX with the missing macros?
