This package has some long-term problems.  The most important is 
that: people who live in multibyte-character regions (including me) 
cannot receive the favor of this package, because of the lack of 
e-TeX-based TeX engines for their native languages.  
So I'm planning to adapt this to conventional TeX engine, 
using the same way of 'Infinite slur ID' (my previous product).
Another is incompatibility with some other extension macrosets.
Motions of the LaTeX2e Project Team may make influence 
on musixuad about register assignment procedure.


Placement of instruments names ... flexibility and freedom 
are important features of MusiXTeX!
For your case, how about this?

\setname1{\hbox to\parindent{\hss Cello}}
\setname2{\vbox{\hbox to\parindent{\hss Piccolo}\hbox to\parindent{\hss 
Clarinet}}}

where \parindent must be hand-tuned, unfortunately.
I think some more hack will enable automatic determination 
of \parindent.  MusiXTeX itself is multi-pass system like 
LaTeX using cross-reference, so a similar way may be applied.
