Sorry, when I wrote that, I was feeling like an idiot. The answer might
be useful in other situations too.

Here are the details.

1. The problem has been reported before on this list (2 May 2011)
and Simon Dreher provided the answer.

2. Which is: "\usepackage{etex} right at the top.
