Another way to get "big" cautionary accidentals:
\makeatletter
\def\largenotesize{\set@largenotesize\comput@specifics
\let\curr@ntsiz@\largenotesize}
\makeatother
Then use {\largenotesize\cna ...} etc. This approach does
allow use of \accshift.
