It's really not that difficult to snake the cable under your shirt and
down to your belt, I wear a light with a battery belt pack all the
time because I have to, I can't wear all that weight on my head--too
hard on the neck.

Neither of the Fenix headlight models have a built in diffuser, and
I think that's where they are lacking. I like the light pattern on
mine, the diffuser addon just rounded it out for when I need one.

Since the battery pack holds 4 AA batteries, and can be belt
mounted, this
lamp is more suited for big stream passage.
If I am going to spend $ 110 or so on a headlamp, it needs to be
better.
