It's really not that difficult to snake the cable under your shirt and down to your belt, I wear a light with a battery belt pack all the time because I have to, I can't wear all that weight on my head--too hard on the neck.

I don't think cavers will want to wear the battery pack on their belt, too much trouble with the cabling getting in the way. I have the Fenix HP10 and I don't think its any heavier than other lights, at least I didn't notice it. My helmet already has 2 backup lights, and a camera bracket on it, so its already heavier than most I think.

Neither of the Fenix headlight models have a built in diffuser, and I think that's where they are lacking. I like the light pattern on mine, the diffuser addon just rounded it out for when I need one.

I know a lot of cavers like products by Fenix.

They have a new headlamp. Cost is around $ 103 plus shipping.

It looks like a good product. I don't like the light pattern as they
show it on their web-site.

Since the battery pack holds 4 AA batteries, and can be belt mounted, this
lamp is more suited for big stream passage.

If I am going to spend $ 110 or so on a headlamp, it needs to be
better.

This lamp can be pre-ordered, and will start shipping in 3 days.
