Batteries have two purposes in life, to store power until you need it andthen 
give it up.

On storage, batteries have oneenemy, and that is self discharge , which slowly 
takes away a battery’s charge. Self -discharge  is lowest when cold, worst when 
hot.

So cool storage is good for a battery. Self discharge is a chemicalreaction; 
reactions areretarded by cold.

On giving up power, that is also a chemical reaction. Heat always accelerates 
andimproves chemical reactions. So batteries will give up the mostcharge 
capacity when warmed and give up less charge capacity when they arecold. So 
warm up batteries when in use to squeeze every last bit of power fromthem.

Everything in moderation. Storing batteries in the refrigerator is probably 
good, howeverfreezing at extreme cold may cause problems with the 
chemicalschanging form some (bad). Heating them when using them means keeping 
them inyour inside pockets or holding them in your hands on a freezing day. But 
don’tset fire to them or hit them with a blowtorch!

Best advice: store cool, usewarm.

There is a Good Inflation and a Bad Inflation

Good Inflation is when the economy booms and productivity and demand areat an 
all time  high. It allows forgrowth and income rise etc.

There is a Bad Inflation whichis caused by Supply deficiency, Mismanagement and 
Oligarchy

Under MMS - Good Inflation often offset the Bad inflation

Today its bad. The GoodInflation is non existent and the bad inflation is 6.75%.

Post the Pandemic, Good Inflation rising is a good thing as it meansbringing 
the economy to heel.Badinflation rising means more bungles.

Mahimna stotra’ asfollows-

Once lord Hari Vishnu decided to worship Shiva by offering 1000 Kamal(Lotuses) 
to Shiva. He sat in front of a shivling and started offering lotuseswhile 
counting them too. Atthe final stage of worship , he realised that he a has 
only offered 999 Lotusesto Shiva and no lotus is left. He has made his mind to 
worship without any kindof interruption, but there was no lotus left; then he 
decided to offer one of his eyes to Shiva insteadof a lotus. As his wife Devi 
Lakshmi oncetold him that your eyes are as beautiful as a Lotus.

And then he took out his one eye and put it on the shivling. Afterwitnessing 
extreme and unparalleled devotion and sacrifice, Lord Shankar appeared in 
frontof lord Hari . Lord Shankar, by his powers, replaced lord Vishnu’s eye and 
alsogave him Sudarshan Chakra as a reward of extreme devotion and sacrifice. 
Andlord Shankar said “ As you have offered your eye as a Lotus; fromnow onwards 
, not only Devi Lakshmi, but all other gods and goddesses andhumans will 
recognize you by the name “Kamal-nayan”.

Since then , we humans also started recognizing him by the name“Kamal-nayan”

Q3         My blood sugar level goesto 170 to 180 within one hour after meal 
and then comes down to 130 in 3 to 4hours after meal, still further goes down

First of all who had suggested to do blood sugar at 1 hr and 3 to 4 
hr?Apparently your blood sugar looks to be normal. Repeat your blood sugar in 
fastingcondition and after 2 hrs of meal. Check. 

Also you can check your HbA1clevels.

My note- Many have these type ofconfusion in mind. Any type ofinformation would 
reach their ears/mind but they won’t believe unless money isspent on Hb1Ac test 
is taken and doctor declare- No diabetics.

patient make?

Few month back a lady came with her 20 day old baby to take aconsultation. She 
was accompanied by her husband and mother in law.

They were from poor family and had came as child was having excessive crying, 
not gainingweight and was becoming weak day by day.

On examination it seemed to be case of late onset sepsis (infection insimple 
terms).I enquired further about birth history and other events. Thefemale told 
that the baby was delivered by ceasrian section and was mainly looked upon 
bygrand mother of the child. She further blamed on her mother in law that she 
was not giving thechild proper care and was keeping the baby with herself 
except for feeding.They started verbal fight in front of me also.

Anyways these types of MIL (mother in law ) and Daughter in law(DIL)blame games 
are common in rural area ,so without paying much heed I asked whether baby was 
takingany medicines. The mother replied sir, plz ask Mother in Law as shewas 
taking care of medicines.

After child birth its very common practise in rural areas that doctorsprescribe 
multivitamins ,nasal saline drops(drops which are put in nose fornasal 
blockage) and some antibiotic eye drop(like ofloxacin,as eye discharge isvery 
common in newborns) and give it to the child mother/father.

Casually I asked how they were using. The grandmother told that she was putting 
nasal salinedrop in eyes and ofloxacin in nose. I was like ,say it again 
.Sheagain told the same thing with pointing towards the medicine. So, the 
dropwhich was to be used for eyes they were putting it nose while the drop for 
eyewas being used for nose.

I literally held my head in disbelief. Antibiotic(ofloxacin ) eye drop ifput in 
nose will not cause much problem. But nasal saline drop contains“NaCl(salt)” in 
weakened (diluted)form ,so you yourself think of the agony thebaby was facing 
as nasal saline drops were being put in his eyes 4 times a day.That poor kid 
,unable to speak was just crying excessively.

My note- There are many who expressbefore doctor well understood but will not 
understood. In Many hospitals the nurses write in Tamil how to use andgive to 
patients.

Ok, I will tell you, Firstly You remember Krishna doesn’t need anyone’s love 
tocomplete, he is complete in himself, secondly his every leela isvery special,

I happen to be the member ofthe 4-member ‘Hypertension Council’of the 
Cardiological Society of India (CSI); the apex committee that chart out 
hypertension guidelinesin India. 

So it sounds funny when the patient says ‘doctor did you know that there is a 
new ‘plant-based’medicine that cures hypertension with just 5 doses; that I 
found outin Google yesterday?’

Clearly, I didn’t.

But I could get a fair idea of his IQ.

One of my relatives called me over phone and sent me her medical details.She 
has lately developedhypertension and diabetes. She managed to check basic blood 
workfrom a local lab which was normal. I tweaked her medication dosage; 
againonline. 2-weeks later, she was still feeling fatigued; which we attributed 
tolockdown blues.

 ‘Hi Doc; Can you call me asap?’

I called.  ‘I was losing a lot ofhair, so I googled and found that it might be 
hypothyroidism. I checkedthyroid; and you know what? It is abnormal’.

Her TSH was above 10; I prescribed thyroxin supplement. Her fatiguedisappeared; 
as did her hair fall.

Both of us were happy; end of story.

I am happy because ‘we’ arrivedat the correct diagnosis.

The problem is, most often neither the doctor, nor the patient search foran 
answer to a complex problem together.

Me, he and we refuse to sit together for coffee.

 ‘The doctor is not good; he failed to make a properdiagnosis.’ The patient 
would say.

 ‘One more fool trying to ‘Google’ his symptoms.’ Comments the irritateddoctor.

Diseases are complex, and sometimes difficult to diagnose. Every issue of‘New 
England Journal of Medicine’ gives a complex case CME where diagnosis 
wasdifficult. All cases are genuine cases from ‘Massachusetts General 
Hospital’which is considered a premier hospital globally.

20% of such cases, the diagnosis is reached ‘at autopsy’.

Yes, medicine is a complexdisease.

The problem is ‘me’ and ‘you’.

The solution is to mix the doctors expertise and the patients clue toarrive at 
the destination of a correct diagnosis, before it is too late.-

Don’t teach the doctor. Don’t laugh at the patient.

Google if you wish; take a printout; add a pinch of salt. Hold it upsidedown. 
If it is still looking authentic; sugar coat it and present it to yourdoctor.

the charges, higher would be the speed, andsometimes better would be the 
offers. So at the time of registering, if youdon’t want the most expensive 
ones, kindly go for the most affordable expensiveones. And if you don’t, then 
kindly don’t complain if very large files takehours to download and if your 
video calls start lagging on Teams and Zooms.

Shoes: Cheap but bad-quality shoes would become a serious pain in yourlegs, 
always remember it

Headphones: Whenever you get opportunity, don’t ever hesitate to opt fora 
expensive headphone over a cheaper one, chances are that if you do so, lateryou

baby or child have?

I had just passed out, after finishing my post-grad in cardiology. A 2-week old 
baby was referredby the paediatrician for a routine echocardiogram. Despite 
being trained mainly inadult cardiology, we were sometimes assigned to do 
screening ECHO for babiesthose days; since Pediatric cardiologists were still a 
rare breed then

The mother carefully placed the flannel-wrapped baby on the Echo table. Iplaced 
the transducer very very gently on the baby’s small chest, careful notto wake 
her up. As unseen bursts of ultrasound waves shot out of thetransducer; 
reflecting back from the small beating heart; I flipped switches,rotated dials 
to adjust the contrast; to make sense of the shadowy image on thescreen. What I 
saw, stunned me.

Inside the ventricle of therapidly beating heart, I could see multiple tumours, 
both inside right and left ventricle; looking likemultiple monsters, waking up 
from sleep, protruding their ugly head. The cuteheart was being hijacked by a 
set of hooligan tumour.

Heart, normally is an organ, which doesn’t allow tumours to grow;excepting rare 
ones called atrial myxoma, which needs to be immediatelyoperated if detected.

I checked, and rechecked the image. The tumours were there for sure.

As I finished the examination and looked at the sleeping baby; she lookedso 
much like my daughter at home-

‘Is there any problem doctor?’

I stammered some kind of an answer; asking the baby’s father to come andtalk to 
me.I explained whatever I could to the shocked parents.Being a newparent 
myself, I could sense their feelings.

‘Let’s try and do whatever bestwe can’ I said.

I wrote a letter to the paediatrician and asked the baby to be referredto a 
Pediatric cardiology center.

 Lost to follow up’ is thecommonest Indian medical diagnosis; and thats what 
happened here. I called the paediatrician; hetold me that the parents shifted 
their residence to somewhere far north andnever came back.

By then I looked up the literature and learned something new.

Tuberous sclerosis is an genetic disease where multiple organ systems canshow 
abnormalities including skin lesions, multiple organ defects andheart-muscle 
tumours called rhabdomyoma.

The severity of disease varies from mild to severe; and surprisingly in many 
babies, the cardiactumour tend to subside spontaneously.

OMG, I just prayed; may be, just may be, the cute baby’s illness is mildand the 
vanishing trick works.

2 years later; ona busy OPD day, the nurse tells me that a patient’s relative 
wants to talk tome.

‘Doctor, you may not remember me, but you had seen my child two yearsback and 
found out that she had some heart tumours. We had to shift to NorthKerala; but 
followed up with local doctors. My wife and me decided to pray for our baby 
every singleday. I think it has worked.’

He brought out a new echo report;the images showing a normal heart anatomy. No 
tumours.

‘Thanks for all the encouragement at a very crucial time; we can’t forgetit’

‘This is my daughters recent photo’. He proudly took it out of hiswallet.

Why the gene TSC1 located in long arm of chromosome 9 runs amok in 
thesechildren is not known, but it is autosomal dominant in transmission. A 
parent, apparentlyasymptomatic, with one copy of a defective gene can transmit 
it to his/heroffspring.

As of now, the Baby is doing well, but the defective TCS1 lurking insideher 
every cell has the potential of creating bad diseases like epilepsy to skin 
tumour, fromkidney disease to lung ailment; anytime.

Just because 1 of the 20,000 functioning gene had decided to be lazy. Onescary 
piece of bad code.

As of now, we wait by the placid shores of Loch-Ness, praying for the monster 
not to return.
