Q4         Is type 2 diabetes reversible?

A4          Dr Maluk Mohamed,Co-Founder, VicePresident (Research) &Head -India
at Twin Health (2018–present)Aug 17

Of course yes. With the advent of technology which has aided inoffering
precision treatment through precisioned treatment factors have madereversal a
truth.

Type 2 diabeteswhich accounts for 90–95% of all diabetes encompasses
individuals who have relative (rather than absolute)insulin deficiency and have
peripheral insulin resistance.

Studies haveindicated that through a low calorie diet and various dietary
programmesnormalizing sugars can be achieved. But however canwe classify those
outcomes as reversal is a debate among professionals.

According to a newconsensus statement from the American Diabetes Association®
(ADA), theEndocrine Society, the European Association for the Study of Diabetes
andDiabetes UK jointly published in Diabetes Care, the Journal of
ClinicalEndocrinology & Metabolism, Diabetologia, and Diabetic
Medicine, respectively, for the first time came out stating that type 2 diabetes
should be considered in remissionafter sustaining normal blood glucose (sugar)
levels for three months or more, without any medications.

Diabetes is seen asa lifelong progressive disease and most physicians and
commoners believe thesame. In 2016, the World Health Organization global report
on diabetesacknowledged that diabetes reversal is possible through weight loss
and reducedcalorie intake. Most agree that a hemoglobin A1c (HbA1c) under
thediabetes threshold of 6.5% for an extended period of time without use
ofglycemic control medications would qualify. Excluding metformin from the
glycemiccontrol medications list, as it has indications beyond diabetes, may
also be aconsideration. International consensus statement outlined diabetes
remissiondiagnosis criteria as a return of the HbA1c to <6.5 that
occursspontaneously or following an intervention and that persists for at least
3months in the absence of usual glucose-lowering pharmacotherapy. However,
anumber of factors can affect HbA1c measurements, including a
varianthemoglobin, differing rates of glycation, or alterations of
erythrocytesurvival that can occur in a variety of disease states. Thus, insome
people a normal HbA1c value may be present when glucose is actuallyelevated, or
HbA1c may be high when mean glucose is normal.

In settingswhere HbA1c may be unreliable, measurement of 24-h mean glucose
concentrationsby CGM has been proposed as an alternative.

My note- I recollect once my nephrologist told me HB1Acis not very reliable  on
certain grounds -

Q5         Is China poor and super poor in therice-farming villages? Why are
villages so poor?

A5          Steven Haddock, B.A. in PoliticalScience21h

China could easilyproduce all the food it needs (mostly rice and wheat) if
there were about 250million people living in rural China. That’s because in the
last 50 years orso, there have been massive improvements in biological
technology, such aswheat that only grows half as tall as normal wheat. This
accounts for the factthat China doesn’t have and probably couldn’t afford all
the technology usedfor producing food in a place like the United States.

This means that China could have a sustainable rural agriculturesystem where
250 million rural dwellers could have a decent living,.

Butthat’s not the case. Rural China has about 750 million inhabitants. There
justisn’t enough money in food production to support that many people to a
decentstandard of living. For nearly 2000 years, China wasstrong because it
needed a huge population to grow food to sustain itself andcreate wealth out of
the surplus. That’s no longer the case - food is cheap andChina’s wealth is now
largely dependent on its manufacturing expertise.

To paythe farmers more would pretty much be subsidization. As such, there’s a
massive flow of people from rural China to urban China, looking for factory
work. There’s also one major demographics problem. Becauseof China’s one child
policy, which only applied to urban areas, and due tourban Chinese preferring
boys, almost all unmarried females live in ruralChina. The same thing happened
in India although it was small families and notofficial government policy that
drove the urban disparities.

If you are a frequent traveller or a good observer then Ism sure you have seen

This mark is used near the points (i.e., where two tracksmerge ) to avoid
collision.

It is strictlyinstructed to the locopilots and guards to stop their train
beyond this foulingmark and it is the duty of the guard to check whether the
train standing onloop line or main line has crossed this mark or not .

In case if thetrain is not beyond the fouling mark ,it may result into

Q9         How can we make our house morecomfortable without using an air
conditioner or a humidifier?

A9          Gopala Krishnan,Former AssistantGeneral Manager 1996-2004 at
Department of Telecom (1966–2004)Just now

There was a periodsay about 8 decades back, in India, there was no electric
power connection tomost homes. In summerwe used hand fans. We grew trees around
homes, many used to sleep on terrace ofhomes in summer enjoying somewhat cool
air.

Before airconditioner units became affordable we used fans.

So growing more trees around, opening windows with properinsect screening, and
using fans we can have some sort of comfort in placeswith high temperature.

Hill stations,do not use air conditioner units. Even places like Bangalore air
conditioner israrely used because of the climate.

Q10              Before passports were invented, could you go to any country
without a problem?

A10        Balaji Viswanathan student of worldhistory. Updated 6y

Passports actually made it much easier to travel betweencountries. It just
replaced a variety of ad hoc stuff.People moving with power have often used
seals, tablets and symbols like thePaiza.

The people movingwithout power [the refugees] were fucked anyway whether in the
past or in thepresent.

The middle category[the vacationing one] didn’t exist for most of history.
Vacations of masses area recent invention and for most of history, people
traveled with a purpose [forreligious or business reasons or as refugees
escaping disaster].

Human beings like many other animals have always beenterritorial and many of
our ancient ancestors didn’t take too kindly whenstrangers invaded their land.
So, you could notwaltz your way into random territory and make it your home.

When you migratedto a new territory, you had to be really prepared if it is
populated. If it isnot populated, it could be because the land was not really
fit or you justturned lucky [for some unforeseen events the previous occupants
left this greatland].

Migrations often were accompanied by wars. When themigrants from central Asia
were flooding all thecivilizations [bring a fall of Rome, Gupta empire,
Sassanids] their migrationwas not smooth. They either had to kill or get
killed. Violence against thesenew strangers was not always treated as a crime.
Unless you had the protectionof something really powerful [a king, priest etc]
your life didn’t have muchvalue.

What the 20thcentury passport did was to enable everyone in the society get the
same movingprivileges as the aristocrats. Passports were standardized, leading
to easierborder controls, and these passports also were available to everyone.
Thevarious international conventions also made it hard to attack the
migratingstrangers/outsiders with impunity.
