Respected Mr Venkitesawaran,
If you want to get removed from iyer 123, I can help you. Before leaving any
group think twice.

Either in the cultural QA or responses by members there are a lot of
information. Similarly other postings. Arguments, responses etc you take in
sportive manner.

No body post in the groups to kill time, either in google groups, or in face
book or in whats app You tube etc.etc.

Members take a lot of time for creation. Nothing is there without a good
content.  After works in domestic front, I make out time for my postings.

invasive surgery and the entire prostate is not removed but smooth passage for
urine alone is set in. Illegitimate surgery alone will land people in any
surgery.
Radical prostatectomy is surgery to remove the entire prostate gland and
surrounding lymph nodes to treat men with localized prostate cancer. A surgeon
can perform a radical prostatectomy using different techniques, including:

   - Robot-assisted radical prostatectomy. The surgeon makes five to six small
incisions in your lower abdomen to remove the prostate. He or she sits at a
console, using instruments attached to a computer-assisted mechanical device
(robot). The robotic device allows a more precise response to movement of the
surgeon's hands.
   - Open radical prostatectomy. The surgeon typically makes an incision in
your lower abdomen to remove the prostate.

Less often, simple prostatectomy may be recommended for men with severe urinary
symptoms and very enlarged prostate glands. Enlarged prostate glands are known
as benign prostatic hyperplasia, or BPH.

Simple prostatectomy is usually performed as a minimally invasive procedure
with robotic assistance. It's not often done as an open procedure anymore.
Simple prostatectomy to treat  BPH doesn't always involve removing the entire
prostate, as in a radical prostatectomy. Instead, this procedure removes just
the part of the prostate that's blocking the flow of urine. The surgery eases
urinary symptoms and complications resulting from blocked urine flow,
including:  A frequent, urgent need to urinate

   - Difficulty starting urination
   - Slow (prolonged) urination
   - Increased urination frequency at night
   - Stopping and starting again while urinating
   - The feeling that you can't completely empty your bladder
   - Urinary tract infections
   - Inability to urinate      Urologists use advanced endoscopic techniques to
address these symptoms without incisions in most cases.

CULTURAL QA 08-2023-11

Q1         What is important in a water purifier before buying them?

A1          Gopala Krishnan, Former Assistant General Manager 1996-2004 at
Department of Telecom (1966–2004)Just now

Nowadays we get closed type R O System  more and with added features. These
added features like adding minerals, passing UV, stages of filtering are looked
in to.

Most companies now provide a large paper filter unit externally through which
the inlet of water is given.

Warranty for power supply unit, free A M C period are looked in to.

Irrespective of the type of R O S purchased, make sure the inlet water is as
best as possible by cleaning over head tank regularly.

This makes the pH value by about 150 and output pH below 50.

In my R O system installed at home, normally the output water to the storing
tank has a pH value around 20 only.

Q2         How much money does turning off the central air save compared to
using a window unit and a ceiling fan together?

A2          Gopala Krishnan, Former Assistant General Manager 1996-2004 at
Department of Telecom (1966–2004)Just now

In India window AC is almost phased out. In large buildings like telephone
exchanges centralized AC systems are provided. Normally two systems are
provided, alternately switched. If at all one system fail, other will be
switched ON.

I have seen in many banking institutions, the engine alternator is provided,
which will take the load of all AC Units also. These alternators can be of
different types.

If at all AC systems fail, fans are switched ON for limited time till AC system
is brought back. AC is important with modern computerized operations anywhere.

UPS battery is provided of sufficient capacity where inverter AC is provided.

My note- Homes equipped with central AC are very few. But there are.

Q3         What happens if the prostate is removed?

A3          HealthyMen, Men’s Health specialist Jul 26

If the prostate is surgically removed, a procedure called a prostatectomy, it
can have several effects on the body. Here are some key points:

1. Infertility:

The removal of the prostate gland leads to the absence of seminal fluid during
ejaculation, resulting in dry orgasms. This means that natural fertility is no
longer possible.

However, prior to surgery, some men may choose to bank their sperm
(cryopreservation) for future use in assisted reproductive techniques, such as
in vitro fertilization (IVF), if they desire to father children.

2. Sexual Function:

The nerves and blood vessels that are essential for erectile function are in
close proximity to the prostate. During a prostatectomy, especially if it’s a
radical prostatectomy (removal of the entire prostate), these nerves may be
unavoidably affected. However, surgeons can attempt nerve-sparing techniques to
preserve erectile function.

Despite nerve-sparing efforts, many men still experience a temporary or
permanent decline in erectile function after surgery. It can take several
months or longer for erectile function to return, if at all, and some men may
require treatments like medications, penile injections, or devices to achieve
erections.

3. Urinary Changes:

After prostate removal, the urethra might undergo some changes due to the
surgery. The prostate surrounds the urethra like a donut, and its removal may
result in changes to the urethral structure or scarring. This can lead to
temporary or permanent urinary incontinence, which means the ability to control
urine flow might be compromised.

Post-surgery, some men may experience urinary leakage when they cough, sneeze,
or engage in physical activities, known as stress incontinence. Others may face
a frequent and sudden urge to urinate, known as urge incontinence.

4. PSA Levels:

Prostate-specific antigen (PSA) is a protein produced by both normal and
cancerous prostate cells. After prostate removal, PSA levels in the blood
should drop significantly, as there is no prostate tissue left to produce PSA.

Elevated PSA levels after a prostatectomy can indicate the presence of residual
prostate tissue or recurrence of prostate cancer. Regular PSA tests are
important for monitoring any potential recurrence.

It’s important to note that while prostatectomy can be an effective treatment
for prostate cancer, it’s not without potential side effects. Each individual’s
response to the surgery can vary, and it’s essential for patients to have a
thorough discussion with their healthcare team to understand the potential
risks, benefits, and long-term implications of the procedure based on their
specific medical history and cancer stage.

My note- I read long back in “kids health”  the topic on removal of enlarged
prostrate is done as last resort when other methods fail. An expert surgeon has
to do the operation since near by glands of urethra is not touched even
accidentally.

Q4         On the kitchen platform in the SE corner, if there is a sink and gas
stove side by side, can there be a strained relationship in the family?

A4          Gopala Krishnan,Former Assistant General Manager 1996-2004 at
Department of Telecom (1966–2004)Just now

Kitchen location based on Vasthu principle is south East/ North East in
southern India. While Kerala it is mostly North East, in Tamilnadu it is South
East. In flats these principles cannot be looked in to.

It is a basic scientific requirement the stove position and sink must be  away
as far as possible. If they are near, while cleaning or water tap switched ON,
sprinkles of water can fall on burner and ignited flame may go OFF.

Other way, if water fallen on burner and the stove is ignited, burner many not
burn. Cleaning water is with chemicals added and may make stains in the stove.

Stove is hot and sink is cool. In certain areas, it is a belief, they should
not be very close. Strained relations in the family because the stove and sink
near, is a thinking with some families.

We follow many such beliefs. An example- number 13 is not considered good in
many countries. They may number 12 A instead of 13.

Q5         What is the hole on the back of passenger airplanes?

A5          Jagrata Banerjee, Studied Aviation Updated Jun 25

Thanks for giving me the opportunity to answer the question…

You may be probably talking about The hole at the tail end of the aircraft??

Yeah, this is the exhaust port of the Hidden Powerhouse of the aircraft. This
is also known as the Auxiliary Power Unit (APU).

Auxiliary power unit is a small turbine engine located at the tail of the
aircraft. It too uses the usual Aviation Fuel from the aircraft's fuel tanks.
The APU is started, when the engines are not running or the plane is not
connected with an external power source.

The APU is switched off after the engines are started, as it is no longer
needed.

Sometimes, the APU is kept on for air-conditioning purposes during take off, as
engines’ bleed air is not used for AC during take off (specially at hot and
high operations).

The APU is again started after the plane exits the runway after landing. This
is because, immediately upon reaching the gate the external power source may or
may not be available and using the engines for running the A/C and electrical
system is not at all economical and is totally impractical. Therefore, the APU
is kept on in such cases.

On a small jetliner, such as an Airbus A320 or a Boeing 737, the APU uses fuel
at the rate of 160–200 kg/h. On the other hand, a big Jumbo Jet, such as a
Boeing 747 or an Airbus A380, uses fuel at 400–500 kg/h.

Functions of APU—

Supply high pressure bleed air to air conditioning packs while on the ground.

Provides electrical power to the aircraft, while the main engines are switched
off.

Provide hydraulic and pneumatic power to start the main engines of the aircraft.

Can be used to start engines while the aircraft is in flight.
