Q1      What aresome practises in India that pose some concern?

The guy working for the cleaning agency came for “Deep cleaning of the toilet.”

They (two of them) locked themselves from inside asthey began the cleaning 
process. They use acids and other chemicals to cleanthe toilet/washroom.

Concern is the safety & health of the workers.They lock themselves from inside 
and are exposed to all types of caustic/acidicfumes.

Another similarpractise:

This young man is standing in this place (under theescalator) since 12 pm at 
the Gurgaon Mall. His duty hours are from 12 noon to 8.30 PM. He do not have a 
seat.In fact no seat has been provided to him. Unable to stand he was fidgeting 
andleaning on the desk as we sat close by sipping tea.

It appears cruel to expect a person to literally beon his feet for 8 hours and 
30 minutes. Why won't the management provide a seatand treat these workers 
decently?

Mostestablishments outsource security & housekeeping services. Those companies 
exploit themto the core. Health safety measures are not employed, protective 
gear is notprovided to them and in some cases, they do not get the weekly offs 
either. Inall shopping malls over India the employers don't provide chair to 
theirworking staff. They work standing on their feet for 8 to 10 hours.

It is trulyinhumane and is not correct.

On our part the least we can do is to ask them to leavedoors open when they do 
the cleaning job and also use the exhaust so that fumesare sent out……apart from 
giving a cup of tea & some biscuits to eat. :-)

I always appreciate the Flynn effect proposed by James RFlynn from New Zealand. 
It states that the IQ of people increases with everypassing generation. 
Absolutely right; your kids are smarter than you.

Younger doctors are more energetic, enthusiastic, smarterand gadget savvy.

Don’t feel offended, if you too (like me) are on thewrong side of 50; we too 
are energetic, smarter compared to our predecessors, but certainly less gadget 
oriented compared to our next-gen.Even the gadgets seem to be intelligent 
enough to sense the same thing; every time I take out the USB driveout of my 
laptop (I do it softly, with utmost care) it emits a loud sound andcouple of 
notification pop up, warning me of stern action if I continueto disconnect 
without proper etiquette; surprisingly no such thinghappen when my son does the 
same thing even though he pulls it out withimpunity like a carious tooth.

This youngresident was no exception. We trained him well for six months and he 
was readyto take independent duty.

During our rounds we saw him rushing to bed number 3being called by the duty 
nurse for a VT (Ventricular Tachycardia)

A VT is a major life threatening heart rhythmdisturbance that makes the heart 
beat very fast and can be fatal if leftuntreated for more than a minute. The 
treatment is to deliver a high-voltage shock using amachine called 
defibrillator.

 ‘Monitor VT’ the nurse called.

Another nurse made the defibrillator ready; chargedit to 200 joules to deliver 
the shock.

The monitor did show a VT.

The patient wasshivering. ‘Goingto arrest’ the resident shouted.

He picked up the pedals of the Defibb, ready todeliver the shock.

I had a doubt. Ifelt the pulse, it was normal.

Hold on for a moment; It is not VT, it is anartefact’.

A shivering had induced artifact (abnormal pattern)in the monitor; which was 
not a VT.

 (For residents- The red arrows marks the normal QRS complex seen through the 
artefact thatlook like VT that clinch the diagnosis)

An injection of IV paracetamol reduced his fever andshivering.

All ended well.

 ‘Always look at the patient, whatever the monitor shows’

In airplane industry they call it a near miss.In theICU another story to be 
told from resident to resident in the cafeteria.

There arereasons why they still don’t throw old doctors out of the hospital 
despiteenough supply of ‘Flynn’

My note- If anyone has a free mind, they can observe theGod given humorous 
writing   ability ofthe doctor even when writing serious subjects.

Have you ever been hungry while traveling in aspeeding highway and you find no 
restaurants nearby.

But suddenly youfind 4 or 5 restaurants all at one place and get confused where 
to go.

Else have you wondered why can't they locate therestaurants every 1km.

The answer is , they just follow “NASH EQUILIBRIUM”

To make you better understand , let me plot this witha simple beach side Pani 
Poori shop story…

A Guy named Ram decided to open a Pani Poori shop atbeach.Let us assume the 
length of the beach to be 1km.

Ram placed hisshop right at the centre of the beach.

His shop had a good reception from the people and thenumber of customers 
increased day by day.

Meanwhile one day his friend Raghav from his villagecalled up and expressed his 
idea in starting a Pani Poori shop, the same likehim.

Ram accepted it and made a deal with Raghav. They split the beach into 
twohalves and located their shop as per the deal.

Both Ram andRaghav had a equal number of customers.This state is called 
Socially optimalsolution.

At this state, the customers need not be travelingmuch to get what they desire. 
But this is not a steady state.

One fine day,Raghav decided to see good profit and broke the deal by shifting 
his shop to thecentre of the beach.

Raghav made good profit that day. Even some of thecustomers of ram have been 
driven towards raghav's shop. He was happy with thatand shifted his shop back 
to the original.

This made Ram angry and he decided to teach Raghav alesson. The next day, ram 
shifted his shop to the centre of the beach.

And result the same. Ram made good profit that day.

This made Raghav angry and he shifted his shop rightnext to Ram's shop.And he 
made profit.

The next day saw ram shifting his shop right next toRaghav's to make profit.

This process wenton for many days. And at one stageboth Ram and Raghav reached 
the centre of the Beach.

Now at thisstage, whoever shifts their shop will end up losing.

This state ofequilibrium is called as NASH EQUILIBRIUM.

At this state, the customers need to be travelingmuch to get what they desire.

As I alreadymentioned, the same can be applied to restaurants in a highway. 
Also to allpetrol bunks, inns, coffee shops, even hospitals.

 “It will be better to keep our competitors nearer to us” That's the moral.

Yes. Its true. Heard of Black gold - the king of spices.You'reright - the 
Pepper.

HistoricallyPepper was one of the most expensive of all spices and was at some 
point evenused as bartermoney. And, India was the solesupplier of this to the 
entire world. Europe was literally gettingbankrupt by importing spices from 
India.

The Dutch, by that time monopolized spice trade inEurope and decided to hike 
its price five shillings up. This proved to be ahistorical blunder.

The unwarranted price raise angered the Englishpeople. The result was the 
formation of the British East India company with amodest £72000 investment. The 
company was oneof the most successful trading firms of all times and at its 
height this singlecompany accounted for almost half of world trade.

Queen Elizabeth signed the permission to trade withIndia on 31st December 1600 
AD and as you know, the rest was history.
